Assumption of Risk & Liability Waiver

By creating an account and accepting this waiver, you acknowledge and agree to the following. Acceptance is recorded with a timestamp, your IP address, and the waiver version shown above.

1. Inherent risk of exercise

You understand that physical exercise carries an inherent risk of injury, including serious injury. You voluntarily choose to participate with knowledge of these risks.

2. Consult a physician

You affirm that you have consulted, or have voluntarily chosen not to consult, a physician before beginning any Fit Patch program.

3. Video instruction is not personalized prescription

You acknowledge that workout videos and written guidance are general fitness instruction, not a personalized medical or physical-therapy prescription.

4. Adaptive needs, prior injuries, chronic conditions

If you have adaptive needs, prior injuries, or chronic conditions, you assume the associated risks and agree to work within your own safe limits and your healthcare team’s guidance.

5. Release

To the fullest extent permitted by law, you release Fit Patch Wellness LLC from claims arising from ordinary negligence. This release does not apply to gross negligence or willful misconduct, and does not override applicable consumer protection laws.
